We provide services for WSIB and other workplace related injuries. However, if injuries are chronic in nature and previously being followed by a different physician, we recommend continued follow-up with that same doctor for the sake of continuity.
If you are planning to come in for a work-place related medical problem, the following forms can be pre-printed and completed prior to your arrival, for your convenience.
(wsib form 8)—This form is to be completed if this is the first reporting of a new injury.
(wsib faf)- This form is to be completed for subsequent visits for a previously reported injury.
